If you’ve ever hired an associate or team member and thought, “This is more work than it’s worth,” you’re not alone.


Clinic owners often step into leadership with the best of intentions, only to find themselves frustrated, resentful, and wondering why their team isn’t helping them grow.


After 20 years of owning clinics and coaching thousands of practitioners, I can tell you this: most of the tension between clinic owners and associates comes down to one thing—mismanaged expectations.


And it shows up in five big mistakes.

1. Expecting Associates to Drive Sales

Here’s the classic misunderstanding: you hire an associate and expect them to “build their practice” inside your clinic.


But the reality is, most associates joined your team precisely because they didn’t want the risk or responsibility of running a business. They’re looking to you for overflow, for reputation, and for your marketing muscle.


That doesn’t mean they get a free pass. Their responsibility is retention. If they can’t hold on to patients, your clinic loses both revenue and credibility.


Your role is to bring patients through the door. Their role is to keep them coming back. Once you get this clear, the frustration on both sides starts to disappear.

2. Letting Associates Sell Only Their Time

If your associates are still selling 30- or 45-minute appointments, they’re stuck in an outdated model.


Patients aren’t buying minutes on a clock anymore—they’re buying transformation.

This is where a Signature Care System changes everything. A clear methodology (whether it’s yours or one they’ve developed) gives associates structure, confidence, and a clear story to tell patients.

3. Confusing Online Presence With Growth

A polished website and Instagram account are nice—but they’re not growth strategies.


Real growth comes from momentum. And momentum, especially for new associates, is built through relationships.


That’s why I recommend the Rule of Five: every associate should cultivate five referral partners—three inside the healthcare space (a chiro, a physio, a compounding pharmacist, for example), and two outside.


When I was practicing, my number-one referral source wasn’t another clinician—it was a divorce coach. Why? Because her ideal clients were the exact women I served: stressed, motivated, and ready to invest in their health. That relationship alone filled my schedule for years.


Your associates need to find their own “divorce coach” equivalent. It’s not about being everywhere online—it’s about building the right five relationships that matter.

4. Capping Your Unicorns

Every clinic has one: the unicorn associate who’s in high demand, fills their calendar with ease, and consistently outperforms expectations.


Too often, owners clip their wings by refusing to let them raise their rates.


Here’s the truth: demand drives pricing. If patients are lining up, rates should follow. Allowing your unicorns to increase their fees doesn’t hurt your clinic—it elevates it. It sets a precedent that excellence is rewarded.


And here’s the bonus: when they raise their rates, you have permission to raise yours too. As the clinic owner, you should always sit slightly ahead of your team on pricing. It positions you as the leader, and it often drives more traffic to your associates because patients have options.


Instead of fearing your unicorns, leverage them. Their growth lifts the whole practice.

5. Avoiding the Hard Conversations

The biggest mistake of all? Silence.


Owners silently expect associates to hustle harder. Associates quietly resent feeling unsupported. And both sides lose.


Clear communication about roles, expectations, and success metrics is what transforms a group of practitioners into a high-functioning team.


If you’ve ever felt like your associates aren’t “pulling their weight,” or if you’ve had staff turnover that left you scratching your head, it’s usually because these conversations didn’t happen early and often enough.


Leadership isn’t about avoiding discomfort. It’s about creating clarity so everyone knows what winning looks like.
